Job Summary
Provide in-home physical therapy as a per diem contractor in the Eastern Suffolk County area. Deliver individualized therapy plans, evaluate patient needs in the home setting, instruct family members and caregivers, document visits accurately, and communicate with patients and the interdisciplinary team. Requires working within geographic area, ability to travel to patients’ homes, and adherence to agency policies and professional standards. Flexible schedule with 1099 and W-2 options; per visit pay range of $70-$90; license in NY or eligibility required.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Physical Therapist Program Graduate
