Job Summary
In-home physical therapy for patients in Central Suffolk County. Per diem position offering 1099 and/or W-2 options. Licensed Physical Therapist in NY or eligible required; Doctorate in Physical Therapy preferred. Evaluate patients, develop individualized plans of care, implement customized therapy programs to restore function, and promote independence. Instruct family members, PTAs, Homem health aides, and caregivers on therapy techniques. Provide timely documentation and communicate with patients, families, and the interdisciplinary team, especially around discharge planning. Travel to patients’ homes within the assigned geographic area with flexible scheduling and administrative support for documentation and compliance.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Physical Therapist Program Graduate
