Job Summary
Patient Coordinator who is bilingual in Spanish to serve as the hub between patients, caregivers, and internal teams. Responsibilities include managing and maintaining patient and caregiver schedules, coordinating care assignments based on caregiver availability and patient needs, restaffing for callouts, confirming completed visits in EVV systems (e.g., HHA Exchange), and assisting with billing and payroll preparation. Requires strong communication, organization, and a caregiver-first, patient-centered approach. Experience in home care or healthcare scheduling is strongly preferred; EVV familiarity is a plus. In-person role based in Boulder, Colorado.
