Job Summary
Part-time Occupational Therapist delivering home-care therapy in Port Washington, NY, within designated geographic area. Provides 1:1 treatment, evaluates patients, develops individualized plans of care based on physician orders, and delivers care aligned with professional standards and agency policies. Educates family members and caregivers on therapy techniques; plans and conducts OT programs to maintain or restore function, relieve pain, and prevent occupational dysfunction in geriatric patients. Performs initial exams, documents visits accurately, and adjusts treatments based on patient progress. Must be willing to travel to patients’ homes within the assigned territory and collaborate with patients, support staff, and management. Required Qualifications
- Licensed Occupational Therapist in the state of New York or eligible
