HOME CARE OCCUPATIONAL THERAPIST OT

Provides patient care services as a HOME CARE OCCUPATIONAL THERAPIST (OT) with direct treatment and follow-up. Evaluates patients new to service using motor evaluation, sensory-perceptual-cognitive evaluation, and/or performance evaluation as indicated by the patient’s condition. Develops a plan of care appropriate to the patient’s diagnosis, establishing measurable goals in compliance with the patient’s physical assessment. Documents patient care data thoroughly and timely, reflecting progress or decline. Coordinates regularly with COTAs and provides monthly supervision of COTA’s patient care. Submits daily/weekly patient visit schedule to the Scheduler and communicates with intercompany contacts. Discusses company and third-party reimbursement policies with the patient/family and treatment plans with the Patient Care Coordinator, physician, and patient/family. Coordinates referrals to appropriate disciplines. Provides in-service education, health care professional education, and public education; supports training of peers and students. Conducts research and assists with orientation of new employees as needed. Willingness to travel as assigned.
