HOME CARE OCCUPATIONAL THERAPIST OT

Provides patient care services with direct treatment and follow-up. Evaluates patients new to service using motor evaluation, sensory-perceptual-cognitive evaluation, and/or performance evaluation as indicated. Develops a plan of care with measurable goals based on patient diagnosis and assessment findings. Documents patient care data thoroughly and timely, reflecting progress. Coordinates regularly with COTA’s and provides monthly supervision of COTA’s patient care. Submits daily/weekly visit schedules to the Scheduler and communicates with Patient Care Coordinator, physician, and patient/family regarding treatment plans and reimbursement policies. Coordinates referrals to appropriate disciplines and provides in-service education and public education as needed. Assists with orientation of new employees and conducts research as required. Requires travel as assigned, licensure and CPR certification, ongoing continuing education, and adherence to background checks/drug-free workplace policies.
