Job Summary
HOME CARE OCCUPATIONAL THERAPIST (OT) provides patient care services with direct treatment and follow-up, evaluates new patients with physician orders using motor/sensory-perceptual-cognitive/ performance evaluations, develops and documents a plan of care with measurable goals, coordinates with COTAs and supervises COTA staff, submits visit schedules to Scheduler, communicates with care teams, discusses reimbursement guidelines with patients/families, coordinates referrals, conducts in-service education and assists with orientation and training; requires a minimum bachelor’s degree in occupational therapy, successful completion of the NBCOT, state licensure, CPR, ongoing CE, and willingness to travel to assigned locations (Jacksonville, NC). The role is Per Diem/PRN Flexible with on-site patient care responsibilities.
Required Qualifications
- Minimum bachelor’s degree in occupational therapy from an accredited school of occupational therapy
- successful completion of the National Certification Exam
- current licensure from the state where therapy will be provided
- CPR certified
- minimum of twelve hours of continuing education annually
- travel willingness as assigned
