Home Care Occupational Therapist- 1099 (Western Nassau County)

Occupational Therapist needed for in-home, per diem work in Western Nassau County. Provide 1:1 OT services within your geographic area, evaluating patients, developing individualized treatment plans based on physician orders, and delivering care aligned with agency policies and the plan of care. Educate family members and caregivers on therapy techniques; perform initial exams and ongoing documentation to track progress; travel to patients' homes within the assigned territory; focus on geriatric patients to maintain or restore physical function and prevent dysfunction. Must be a licensed OT in New York (or eligible) with graduation from an OT program; home health experience preferred but not required, with mentoring available. Flexible schedule with both 1099 and W-2 options; pay ranges per visit are provided, with compensation contingent on factors including experience and geography. Duties include 1:1 treatment, documentation, and adherence to professional standards while delivering compassionate in-home care.
