Job Summary
Occupational Therapist, in-home per diem role in Brooklyn, NY. Provide 1:1 occupational therapy services to geriatric patients within your designated geographic area, travel to patients' homes, evaluate and develop individualized treatment plans, educate family members and caregivers, document outcomes, and adapt treatment to maximize function. Must be a Licensed OT in NY or eligible, with willingness to travel to patients’ homes; flexible scheduling; 1099/W-2 options available; pay is per visit ($70-$90). Focus on patient care, professional standards, and collaborative care with clinicians, caregivers, and management.
Required Qualifications
- Licensed Occupational Therapist in the state of New York or eligible
- OT program graduate
- Willingness to travel to patients' homes within assigned geographic territory
- Ability to provide 1:1 treatment
- Documentation of exams and treatment plans
- Strong communication skills
- Experience in home health preferred but not required (mentorship provided)
- Per Diem or contract work arrangement
