Job Summary
Lead the implementation, operations, and compliance of Intensive Case Management, Clinical Assessment & Eligibility, and State Home Care programs in accordance with MassHealth and Executive Office of Aging & Independence requirements. Directly supervise nursing and administrative staff while serving as the primary liaison to state agencies for assigned initiatives. Provide clinical leadership, coaching, and training on complex care coordination, fall prevention, and dementia care to ensure regulatory adherence and high-quality service delivery. Oversee enrollment, eligibility, and program spending for waiver and enhanced Home Care programs, conducting nursing assessments, home visits, and discharge planning for medically and behaviorally complex consumers. Partner with hospitals, skilled nursing facilities, and community providers to support safe care transitions and analyze program data to drive operational improvements and reduce discharge barriers.
Required Qualifications
- Graduation from a School of Nursing accredited by the National League for Nursing
- B.S.N.
- Valid license as a registered nurse in the Commonwealth of Massachusetts
- At least three years of full-time or equivalent experience as a registered nurse in a community setting
- At least one year of supervisory, administrative, or managerial experience
- Previous experience with the older adult population
- Previous experience with mental health
- Previous experience with care coordination
- Previous experience with community health settings
- Knowledge of the State Home Care system
- Knowledge of MassHealth LTSS programs
- Knowledge of community resources
- Must receive certification in Habilitation Therapy within the first 6 months of hire
- Valid driver's license
- Must be able to travel to consumers' homes, area hospitals, and nursing homes via public transportation or by car
- Meets both CORI/SORI and public health screening requirements

- Additional experience beyond one year of supervisory, administrative, or managerial experience
- Clinical supervision experience
- Aging Services Access Point (ASAP) experience
- Experience with Wellsky
- Experience with compliance monitoring and tracking
- Ability to interpret regulatory and business processes and develop and implement policies and procedures required
