Job Summary
Conduct in-home patient assessments, develop individualized care plans, and administer medications and treatments as prescribed. Monitor and document patient health status, coordinate with physicians and therapists for continuity of care, and educate patients and families on health management and safety. Provide wound care, IV therapy, and other skilled nursing interventions while maintaining accurate clinical documentation in compliance with regulations. Requires an active RN or LVN license in California, CPR/BLS certification, and valid driver's license. Offers flexible scheduling, health and vision insurance, and performance-based bonuses.
Required Qualifications
- Active RN or LVN license in the state of California
- Previous home health, home care, or community nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Valid California driver's license and reliable personal transportation
- CPR/BLS certification required
- Compassionate, self-motivated, and comfortable working independently
