Home Care Marketing Representative

Community Outreach Representative to promote Elite Home Health Care's services in NYC and downstate NY. Responsibilities include building and managing referral relationships with hospitals, nursing homes, physician offices, rehabilitation facilities, adult day health programs, social workers, case managers, and community organizations; developing referral sources to drive census growth; promoting services through outreach events, health fairs, presentations, and community-based activities; conducting home visits and enrollment support; following up on referrals; maintaining ongoing communication with partners; reporting activity to management; representing the agency professionally and identifying market growth opportunities. Requirements include at least 1 year in sales, healthcare marketing, or community outreach (home care experience preferred), knowledge of MLTC/Medicaid preferred, strong communication and interpersonal skills, ability to travel daily within the territory, self-motivation, and bilingual in Spanish and English preferred.
