Job Summary
Oversee day-to-day operations of the Home Care department to ensure high-quality, safe care meets state and federal regulations. Direct Personal Care Supervisors, schedulers, and aides while managing schedules and prioritizing staffing needs based on enrolled participant requirements. Conduct quarterly supervisory home visits, perform initial and annual competency assessments, and coordinate plan of care development with the Interdisciplinary Team. Evaluate staff performance and communicate participant changes during required IDT meetings to support 24-hour care delivery. Requires C.N.A. certification, one year experience with the frail elderly, and two years in management. Schedule is Monday-Friday 8:00am-5:00pm with occasional weekends.
Required Qualifications
- Bachelor's Degree or equivalent education and experience
- Minimum requirement of C.N.A. certification
- Current State Specific certification as a Licensed Practical Nurse, Registered Nurse, Occupational Therapist, Physical Therapist or other licensed professional
- Cardiopulmonary Resuscitation (CPR) certification
- A minimum of one year's experience in working with the frail or elderly population
- Minimum of two (2) year's management experience in a hospital, nursing home or community-based setting
- Must have a valid driver's license
- Proof of insurance
- Have reliable means of transportation
Desired Qualifications
- Current State Specific certification as a Licensed Practical Nurse, Registered Nurse, Occupational Therapist, Physical Therapist or other licensed professional preferred
- Home health experience preferred
