Home Care Manager

Job Summary
Implement home care services for program participants by completing safety reviews, coordinating durable medical equipment, and assessing needs for in-home personal care. Lead the home care team, providing oversight, conducting supervisory visits, and delivering regular performance feedback to home health aides and schedulers. Develop individualized care plans and collaborate with nurses, social workers, and therapists to coordinate participant needs. Complete home care assessments at enrollment and reassessments, present recommendations to the Interdisciplinary Team, and monitor contracted home health aide services. Ensure schedules are staffed per Care Plans, verify timesheets and mileage logs for payroll, and participate in quality improvement initiatives. Travel throughout the New Orleans metropolitan area to perform home visits, requiring a valid driver's license and vehicle.
Required Qualifications
- Bachelor's degree in nursing or healthcare related field
- One year of experience working with the frail and elderly population
- At least one year of prior supervisory experience
- Valid LA driver's license
- Vehicle
- Clean driving record
- CPR and First Aid Certification
- Must be a minimum of 18 years of age
- Ability to travel to and perform home visits throughout the New Orleans and the metropolitan area
- Must possess the ability to make independent decisions when circumstances warrant such action
- Must be able to deal tactfully and patiently with personnel, participants, family members, visitors, government agencies/personnel, and the public
- Must have the cognitive ability to process and perform tasks for participant safety
- Must be able to move intermittently throughout the workday
- Must be able to lift, push, pull, and move a minimum of 25 pounds
Desired Qualifications
- Will consider LPN, COTA, MA, or other discipline with related experience in lieu of education requirements
