Job Summary
Provide professional nursing care to patients in their homes, managing infusion therapy and ensuring compliance with physician orders. Conduct assessments, medication reconciliation, and care plan updates while collaborating with physicians and utilizing secure messaging for insurance authorizations. Document all clinical decisions, interventions, and teaching strategies in the EMR and report care problems to management. Maintain visit frequency within authorization ranges and communicate pertinent patient information during care load transfers. Complete routine assignments independently and adhere to organizational standards, including lifting 50 lbs and standing 8 hours daily.
Required Qualifications
- Associate's Degree in Nursing or Diploma Certificate in Nursing
- Minimum of one (1) or more years of RN Experience
- Ability/Experience with Inserting IVs
- NJ State Professional Registered Nurse License
- BLS AHA Basic Health Care Life Support HCP Certification
- Successful Completion of Infusion Test within probationary period with grade of 80% or higher
- Excellent written and verbal communication skills
- Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms
- Lifts a minimum of 50 lbs.

Desired Qualifications
- With previous and recent IV experience (preferred) within one (1) year: Demonstration with preceptor inserting peripheral line, accessing ports, PICCs and central lines
- Without previous experience or experience not within one (1) year: Will complete a vascular access device (VAD) course through HMH and follow up practicum experience in HMH outpatient infusion setting and return demonstration with preceptor inserting peripheral line, accessing ports, PICCs and central lines
- Bachelor of Science in Nursing Degree (BSN)
