Job Summary
The Home Care Hospice Liaison position drives awareness and referrals for hospice and home care services by building and maintaining relationships with hospitals, physicians, skilled nursing facilities, and community partners. Responsibilities include educating healthcare providers and the community about hospice services, generating referrals to support census growth, conducting routine visits to hospitals and clinics, coordinating with the clinical team for smooth patient admissions, maintaining referral activity records, participating in community events, and identifying new business development opportunities within the assigned territory. Requires a valid California driver’s license and reliable transportation; prefers a Bachelor's degree in healthcare, marketing, or related field, and healthcare marketing/sales experience. Knowledge of hospice services and Medicare guidelines, strong communication and organizational skills, and HIPAA/compliance awareness are essential.
Required Qualifications
- Valid California driver’s license with proof of current automobile insurance and reliable transportation required.
- Two years of healthcare marketing, sales, or business development for Home Care Services.
- Established referral relationships and knowledge of hospice services strongly preferred.
- Knowledge/Skills/Abilities: Strong communication and interpersonal skills; ability to develop and maintain referral relationships; experience with community outreach and territory growth; strong organizational and time management skills.
- Experience with Medicare guidelines and HIPAA compliance.
