Job Summary
Conduct on-site client evaluations, develop service plans, and perform daily field visits while recruiting, hiring, training, and supervising attendants. Complete daily visit reports and submit forms to the office within 48 hours of each visit. Correct and resubmit error rejections promptly and maintain supervisor files. Communicate regularly with management regarding department issues and adhere to federal and state compliance regulations. Participate in staff meetings, community relations activities, and special projects to promote organizational policies. Maintain confidentiality of client and staff information while demonstrating flexible time management and a positive attitude.
Required Qualifications
- Two years full time study at an accredited college or university
- Two years full time study at an accredited college or university, individuals hired with a high school diploma or equivalent may substitute one year of full-time employment in a supervisory capacity in a healthcare facility agency or community based agency for each required year of college
Desired Qualifications
- PHC, MC experience preferred
