Job Summary
Provide nursing care to home health patients under physician plan; perform assessments and nursing procedures; review plan with RN prior to initial visit; direct patient care using nursing skills and judgment; recognize changes in patient condition and report to RN/MD; report patient findings to primary RN; identify and report equipment malfunctions; implement components of the health care teaching plan; utilize materials and teaching strategies to accommodate patient/caregiver learning needs; complete documentation per agency and regulatory requirements; perform related duties as essential functions under ADA.
Required Qualifications
- Graduate of an accredited school of practical nursing or of a Nursing Vocational or Nursing Technical school required.
- Current License to practice as a Practical Nurse in New York State required, plus specialized certifications as needed.
- Prior acute care experience strongly preferred.
