Job Summary
Coordinate incoming calls regarding physician orders, participant questions, and referrals while communicating schedule changes and test results with nursing, therapy, aides, and physicians. Conduct in-home care assessments and partner with Home Care Services staff to track delivery and monitor scheduling hours. Collaborate with interdisciplinary teams to arrange personal care, assist with staffing and scheduling activities, and support enrollment of prospective participants. Participate in end-of-life care coordination and support. This hybrid role requires onsite presence for team meetings and assessments, offering flexible scheduling based on your desired structure.
Required Qualifications
- Healthcare/medical licensure or equivalency
- At least three (3) years of professional experience
- Minimum of three (3) years of case management or nursing experience in a clinical or home setting with a frail or elderly population
- Proof of vaccination or a vaccine declination form
Desired Qualifications
- Bachelor's degree
- Nursing knowledge and training necessary to treat frail, elderly participants and care for complicated clinical conditions
- Care planning and management experience in a home care environment
