Job Summary
RN Home Care Coordinator position in LifeCircles PACE (Holland, MI) coordinating in-home care and support across clinic, home, and facility settings.Responsibilities include assessing participants’ acute and chronic needs, providing treatments within scope, monitoring vitals and lab specimens, assisting with ADLs, coordinating care plans with the interdisciplinary team, educating participants and caregivers, documenting care and statuses, maintaining a safe clinical environment, assisting with equipment maintenance and supply inventory, participating in on-call rotation, and maintaining licensure and certifications. Requires active Michigan RN or LPN license, valid driver’s license with reliable transportation for home visits, and CPR certification. Required Qualifications
- Current, active Practical Nurse (LPN) license in the State of Michigan, required.
- Current, active Registered Nurse (RN) license in the State of Michigan, required.
- Valid Michigan driver’s license and reliable transportation to conduct home visits.
- Minimum of 1 year of experience working with a frail or elderly population, required.
- CPR certification through the American Heart Association (AHA), or willingness to obtain it through LifeCircles.
