Home Care Coordinator

Job Summary
Conduct initial and ongoing assessments of participants' home care and safety needs, then develop and update individualized home care plans in collaboration with the Interdisciplinary Team. Coordinate and oversee home care services, supplies, and equipment while monitoring participant status to adjust services as conditions change. Communicate regularly with participants, families, caregivers, and care providers, and coordinate care across home, assisted living, adult foster care, and nursing home settings. Maintain accurate, timely documentation in the electronic medical record and ensure regulatory compliance. Participate in quality improvement initiatives, audits, and program development activities to promote and protect participant rights, dignity, and quality of life. Travel to participant homes and community facilities as needed. This full-time, in-person role in Traverse City, Michigan, offers a regular weekday schedule with occasional off-schedule or weekend work.
Required Qualifications
- High School Diploma or equivalent
- Valid CPR certification
- Michigan Driver License
- current auto insurance
- acceptable driving record
- At least 1 year of experience working with frail or elderly population
