Job Summary
The Home Care Coordinator role involves coordinating and implementing home care services for post-acute participants within the Franciscan Health PACE program. Responsibilities include arranging home care, DME and supplies; providing intake data and background information to the multidisciplinary team; assessing homecare needs and contributing to care plans; collaborating with IDT members to deliver services; and coordinating scheduling and education of outside vendor partners for applicable post-acute requirements. Strong emphasis on communication with care teams, planning, and coordination to ensure seniors receive comprehensive home-based support.
Required Qualifications
- High School Diploma/GED - Required
- 1 Year Working with Frail or Elderly - Required
