Job Summary
Assess home environments and functional capabilities of participants, then coordinate ADL, IADL, and DME services while ensuring continuity of care. Facilitate discharge from hospital or nursing home and monitor quality of care provided to participants. Supervise CNA/caregiver staff and collaborate with occupational therapists to address sensitive issues. Requires current Practical Nurse license, one year experience with frail or elderly populations, and working knowledge of community care management. Full-time daylight shift from 8:30 a.m. to 5:00 p.m. Lutheran SeniorLife serves western Pennsylvania residents through comprehensive programs including home health, personal care, and memory care.
Required Qualifications
- Graduated from an accredited school of Nursing
- current Practical Nurse license; in good standing, from the Commonwealth of Pennsylvania
- Minimum of one (1) years' experience working with a frail or elderly population
- Working knowledge of the interdisciplinary model of community care management
- Experience working with cognitively impaired seniors
- Must be able to relate well to seniors and their families to deal with sensitive issues and facilitate problem solving
- Must be flexible and able to change easily
- Working knowledge of computers, Microsoft Word and Excel
