Home Care Coordinator

Home Care Coordinator oversees and manages delivery of home support and personal care services, serving as the main liaison among clients, caregivers, service coordinators, and families. Responsibilities include conducting client intake and assessments, developing and updating individualized care plans, scheduling and assigning caregivers based on needs and location, monitoring service delivery for compliance with care plans and regulations, coordinating with external providers to facilitate transitions of care, supervising and coaching caregivers, and ensuring accurate documentation and compliance with HIPAA and licensing requirements. Requires strong organizational and multitasking abilities, excellent communication, knowledge of home care regulations, and proficiency with electronic health records and scheduling software.
