Job Summary
Provide personalized, one-on-one in-home care to clients, including assistance with activities of daily living, safe transfers, bathing/grooming support, mobility-device help, light housekeeping (e.g., changing linens, laundry, keeping living areas hazard-free), medication reminders and wellness checks. Maintain accurate care notes and promptly share changes with the RN or Care Supervisor. Engage clients with reading, reminiscence, and simple chair exercises to support activity and well-being. Requires completion of an accredited Home Health Aide program (or equivalent), at least 1 year of paid home-health/hospice/facility experience, and a valid driver’s license with reliable transportation; strong written and verbal communication skills for updating digital care logs.
