Job Summary
Provide in-home, one-on-one care to patients following a personal care plan. Assist with activities of daily living (moving in/out of beds, baths, wheelchair transfers, mobility assistance), manage housekeeping tasks (laundry, cleaning, etc.), and support personal care. Maintain records of patient care, progress, or problems and report observations to the supervisor. Engage patients to support mental health and alertness through conversation or reading aloud. Provide medication reminders and facilitate therapeutic activities or exercises. Requires Graduation from an accredited Home Health Aide program and High School Diploma or GED; one year of professional experience; driver's license; CPR certification preferred.
