Job Summary
Provide one-on-one home-based care to patients, assisting with activities of daily living (transfers, baths, mobility), housekeeping tasks, meal support, medication reminders, and engagement activities. Maintain patient care records, report observations to supervisor, build rapport with patients, follow a one-on-one care plan. Requires completion of an accredited Home Health Aide program, High School Diploma or GED, and one year of prior professional experience; valid driver's license; CPR preferred.
