Job Summary
Provide compassionate in-home patient care on a one-on-one basis following a patient care plan. Assist patients with activities of daily living (moving in/out of beds, baths, wheelchairs), perform housekeeping and personal care tasks (changing bed linens, laundry, cleaning), monitor and report patient condition, maintain records of care, and engage patients to support mental alertness. Administer or remind about medications, and support patients with movement and mobility, therapeutic activities, and basic exercise. Requires completion of an accredited Home Health Aide program, a high school diploma or GED, at least one year of professional experience, and a valid driver's license; CPR certification is preferred. Strong verbal and written communication skills are essential, as is attention to detail and the ability to build genuine relationships with patients.
