Job Summary
We are seeking a Home Care Aide to work with patients in their homes under a one-on-one care plan. Responsibilities include assisting with daily activities (moving in and out of beds, baths, wheelchairs, or automobiles), performing light housekeeping (changing bed linens, laundry, cleaning, personal care), maintaining patient care records and reporting observations to a supervisor, building relationships with patients, providing medication reminders, and engaging patients in exercises or activities. Strong verbal and written communication are required. Prior Home Health or Hospice experience is a plus. Qualifications include graduation from an accredited Home Health Aide program, a High School Diploma or GED, one year of prior professional experience, a driver’s license, and CPR certification preferred.
