Job Summary
Provide one-on-one home health care to patients in their homes under an individualized care plan. Assist with daily activities including transfers, bathing, linen changes, laundry, and personal care; maintain records of patient care and observations, report to supervisor; engage patients to support mental well-being through conversation or reading aloud; administer medications and immunizations; lead patients in exercises and activities to promote health. Requires completion of an accredited Home Health Aide program, a High School Diploma or GED, at least one year of professional experience, and a New Jersey Certified Home Health Aide certification (driver’s license and CPR certification preferred).
