Home Care Aide

Provide in-home patient care on a one-on-one basis per a care plan, assisting with daily activities (transfers, bathing, mobility, meals) and maintaining a clean, healthy home environment. Deliver personal care, monitor patient condition, and report observations to the supervisor. Build rapport with patients through conversation and engagement, assist with medications/immunizations, and facilitate exercises or activities to support mental and physical well-being. Requires completion of a Home Health Aide program, a high school diploma or GED, one year of professional experience, and successful TB screening and background check.
