Home Care Aide

Provide one-on-one home health care to patients in their homes, assisting with daily activities (moving in/out of beds, baths, wheelchairs, or automobiles), changing bed linens, light housekeeping, and personal care; maintain records of patient care and report observations to the supervisor; build relationships with patients through conversation or reading aloud to promote mental engagement; administer medications and immunizations; engage patients in exercises or activities as part of the care plan; strong verbal and written communication is essential; prior Home Health or Hospice experience is a plus.
