Home Care Aide

Provide one-on-one in-home care to patients following a care plan, assisting with daily activities (bathing, moving in/out of beds, personal care) and maintaining a clean, healthy home environment. Maintain records of patient care and progress, report observations to supervisor, build relationships with patients, administer medications and immunizations as prescribed, and engage patients in exercises or activities. Requires completion of an accredited Home Health Aide program, a high school diploma or GED, and at least one year of professional experience; TB screening and background check required. Prior Home Health Aide or hospice experience is a plus.
