Job Summary
Provide one-on-one in-home patient care following a care plan, including assistance with daily activities (transfers, baths, linens, light housekeeping), personal care, and documentation of patient condition; build relationships, engage patients with conversations/read-aloud, and support activities to promote mental alertness; may administer medications and immunizations; prior Home Health Aide experience is a plus; requires completion of an accredited Home Health Aide program, a high school diploma or GED, and a TB test with a background check.
