Home Care Aide

Provide one-on-one home health care to patients in their homes, assisting with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles, and performing personal care tasks; maintain records of patient care and progress; follow a one-on-one care plan and report observations to supervisor; build relationships with patients through conversation or reading aloud to support mental health; administer medications and immunizations as needed; engage patients in exercises or activities; strong verbal and written communication skills and prior Home Health Aide experience preferred.
