Home Based Primary Care Physician (MD/DO)

Provide high-quality, patient-facing care for Medicare Advantage members with complex medical, functional, and social needs across New Jersey homes. Conduct comprehensive in-home assessments evaluating medical, behavioral, and social requirements while managing a panel of medically complex patients. Lead post-discharge visits to support safe recovery, identify early clinical status changes, and intervene proactively to reduce avoidable hospitalizations. Collaborate with nurses, care managers, pharmacists, and specialists to coordinate care, manage chronic conditions, and build trusted relationships with patients and families. Document accurately to support continuity and use clinical technology tools to guide evidence-based decisions.
