Job Summary
The Home-Based Primary Care Nurse Practitioner delivers comprehensive geriatric and extended care within the HBPC program, performing advanced clinical practice, care planning, and coordination. Responsibilities include developing individualized care plans, leading interdisciplinary collaboration, ordering and interpreting labs and diagnostic tests, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, and providing education to Veterans and their families. The NP conducts home visits, Veteran Video Connection visits, and telephone visits to Veterans with complex needs, aiming to maximize health outcomes and reduce urgent care/emergency visits, in alignment with ICARE values (Integrity, Commitment, Advocacy, Respect, Excellence). Preferred experience includes 2-3 years as a Nurse Practitioner with diverse patient populations, including Veterans and geriatric/high-risk patients.
Required Qualifications
- Licensed as a Nurse Practitioner
- National certification as NP
- Ability to provide home-based primary care services
