Job Summary

Nurse Practitioner provides geriatric and extended care within the Home-Based Primary Care program, delivering comprehensive care to Veterans in the home setting. Responsibilities include developing individualized plans for disease management, leading and collaborating with the interdisciplinary team, ordering and interpreting labs and diagnostics, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, educating patients and families, and adjusting care plans to optimize outcomes. Care is delivered in the home, via Veteran Video Connection (VVC) visits, and telephone visits, with a focus on quality, timeliness, and alignment with ICARE values (Integrity, Commitment, Advocacy, Respect, and Excellence). Other duties as assigned.

Required Qualifications

  • Nurse Practitioner license
  • National certification as a Nurse Practitioner
  • Advanced education (MSN/MS equivalent)
  • Ability to provide care within HBPC program (Geriatrics/Extended Care)
  • Prescriptive authority as appropriate
  • Interdisciplinary collaboration skills
