Job Summary
Provides advanced clinical care as a licensed Nurse Practitioner in Geriatrics and Extended Care within the Home-Based Primary Care (HBPC) program. Delivers care across health promotion, prevention, geriatric primary and palliative care in the home, including home visits, Veteran Video Connection (VVC) visits, and telephone visits. Orders and interprets labs and diagnostic studies; prescribes pharmacologic and non-pharmacologic interventions; initiates referrals to specialty services; educates patients and families; and adjusts the care plan to achieve optimal outcomes. Collaborates with an interdisciplinary team, demonstrates leadership, and upholds ICARE values (Integrity, Commitment, Advocacy, Respect, Excellence). Preferred experience includes 2-3 years of NP experience with diverse Veteran populations.

- Nurse Practitioner licensure
