Job Summary
Provides advanced clinical practice as a Nurse Practitioner in Geriatrics and Extended Care. Delivers home-based primary care to Veterans, including health promotion, disease management, and palliative care. Performs home visits, Veteran Video Connection visits, and telephone visits; orders and interprets labs and diagnostic studies; prescribes pharmacologic and non-pharmacologic interventions; initiates referrals to specialty services; educates patients and families; develops individualized plans of care and adjusts as needed. Collaborates with an interdisciplinary team, exemplifies ICARE core values (Integrity, Commitment, Advocacy, Respect, Excellence), and delivers care within the HBPC program to Veterans with complex needs. Requires licensed independent practice with advanced education and national certification. Preferred experience includes working with diverse populations, Veterans, geriatrics, and high-risk patients, with 2-3 years NP experience.
