Job Summary
Provides advanced clinical practice as a licensed Nurse Practitioner in Geriatrics and Extended Care, delivering Home-Based Primary Care within the HBPC program. Makes home visits and Veteran Video Connection (VVC) visits and telephone visits to Veterans with complex needs; orders labs and diagnostic studies; prescribes pharmacologic and non-pharmacologic interventions; initiates referrals to specialty services; educates patients and families; assesses and adjusts care plans to achieve optimal outcomes; collaborates with the interdisciplinary team and upholds ICARE values. Delivers care in the home of Veterans with complex needs and addresses health promotion, health prevention, geriatric and primary care, and palliative care as part of the HBPC program. Includes duties such as home visits, VVC visits, and telephone visits.
Required Qualifications
- Licensed Nurse Practitioner with national certification
- Advanced clinical practice
- Ability to provide home-based care including health promotion, disease management, and palliative care
- Experience with elderly and geriatric populations
- Collaboration with interdisciplinary team
- Ability to order/interpret labs and diagnostic studies
- Prescribe pharmacologic and non-pharmacologic interventions
- Provide education to patients and families
- Home visits and remote communications (VVC/telephone) as part of HBPC program
