Home-Based Primary Care Nurse Practitioner

Nurse Practitioner in the Home-Based Primary Care/Geriatrics and Extended Care program delivers patient-centered, evidence-based care in the home or via VVC/telephone visits for Veterans with complex needs. Responsibilities include developing individualized care plans for population health and disease management, leading and collaborating with an interdisciplinary team, ordering and interpreting labs and diagnostics, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, educating patients and families, and adjusting plans to optimize outcomes. Provides care within HBPC focused on health promotion, health prevention, geriatric and primary care, and palliative care; delivers care in Veterans’ homes and via VVC/telephone visits, with emphasis on reducing unnecessary urgent care and ER visits and upholding ICARE values of Integrity, Commitment, Advocacy, Respect, and Excellence.
