Job Summary
Home-Based Primary Care Nurse Practitioner provides advanced clinical practice in Geriatrics/Extended Care, delivering evidence-based primary and palliative care in the home setting to Veterans, with leadership and collaboration in an interdisciplinary team. Responsibilities include developing individualized care plans, ordering and interpreting labs and diagnostics, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals, educating patients and families, and delivering care across home visits, VVC visits, and telephone visits within the HBPC program. Emphasizes quality, timely care and ICARE values; prefers 2-3 years NP experience with diverse Veteran populations. Location: Boston, MA.
Required Qualifications
- Nurse Practitioner licensed
- national certification preferred
- experience with geriatrics/high-risk populations preferred
