Job Summary
Home-Based Primary Care Nurse Practitioner providing comprehensive geriatric and extended care in Veterans Affairs HBPC program. Delivers evidence-based, patient-centered care in patients' homes, including health promotion, disease management, and palliative care; performs labs/diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals, conducts VVC and telephone visits, educates patients/families, and collaborates with an interdisciplinary team to optimize outcomes.
Required Qualifications
- Licensed Nurse Practitioner (NP)
- National certification as NP or equivalent
- Ability to practice independently within scope of certification
