Job Summary
Provides advanced clinical care as a Nurse Practitioner in the Home-Based Primary Care program for Veterans in Geriatrics/Extended Care. Develops and implements individualized plans to achieve optimal health and disease management, orders and interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals, and provides education to patients and families. Delivers care in veterans' homes, conducts Veteran Video Connection visits and telephone visits, and collaborates with an interdisciplinary team to maximize health outcomes and reduce urgent care needs, while exemplifying ICARE values (Integrity, Commitment, Advocacy, Respect, Excellence). Job responsibilities include health promotion, disease management, geriatric and primary care, and palliative care, with opportunities to lead and coordinate care for complex patients.
Required Qualifications
- Nurse Practitioner license
- National certification
- Current state license in California
- 2-3 years Nurse Practitioner experience (preferred)
- Graduate from accredited NP program
- Practice within Geriatrics/Extended Care
- Ability to develop individualized care plans
- Experience delivering home-based primary care
- Strong interdisciplinary collaboration skills
