Job Summary
Nurse Practitioner provides comprehensive geriatric and palliative primary care in the Home-Based Primary Care program for Veterans, leading individualized care plans, ordering and interpreting labs and diagnostic studies, and prescribing pharmacologic and non-pharmacologic interventions. Works with an interdisciplinary team to deliver care in the home, including home visits, Veteran Video Connection visits, and telephone visits. Provides patient and family education, makes referrals to specialty services as needed, and adapts plans to optimize outcomes while upholding ICARE values. Preferred experience includes caring for diverse Veteran populations and high-risk patients; 2–3 years NP experience is noted.
Required Qualifications
- Nurse Practitioner license
- Advanced education and national certification as Nurse Practitioner
- Experience in geriatrics or extended care preferred (2-3 years)
