Job Summary
The Home-Based Primary Care Nurse Practitioner delivers NP-led geriatric and extended care services in the HBPC program, providing comprehensive, evidence-based care to Veterans in their homes. Responsibilities include developing individualized plans for disease management, leading and collaborating with the interdisciplinary team, ordering and interpreting labs and diagnostics, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, educating patients and families, and adjusting plans to optimize outcomes. The role emphasizes visiting Veterans at home (and via VVC/telephone visits) to maximize health and minimize urgent care, aligned with ICARE values and a focus on geriatric, primary, and palliative care within the VA system. Preferred experience includes 2–3 years of NP practice with diverse and high-risk patient populations.
Required Qualifications
- Nurse Practitioner license
- National certification as Nurse Practitioner
- Licensure in Wisconsin (if required)
