Job Summary
Nurse Practitioner provides home- and community-based primary and geriatric care within the VA HBPC program, delivering evidence-based care in patients’ homes, including health promotion, disease management, and palliative care; conducts home visits, VVC/telephone visits, orders and interprets labs and tests, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals, educates patients and families, and collaborates with an interdisciplinary team while upholding ICARE values.
Required Qualifications
- Licensed Nurse Practitioner with national certification
- Independent practice authority (as NP)
