Job Summary

Provides home-based primary care for Veterans within geriatrics and extended care, delivering evidence-based, patient-centered care in the home. Develops and implements individualized care plans to optimize health and disease management, orders and interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and educates patients and families. Collaborates with an interdisciplinary team, delivers care via home visits, Veteran Video Connection (VVC) and telephone visits, and supports health promotion, prevention, geriatric and palliative care in the HBPC program; adheres to ICARE values and strives to maximize patients' health outcomes.

Required Qualifications

  • Nurse Practitioner license
  • national certification in appropriate specialty
  • advanced clinical practice training
  • ability to provide care in home-based setting
