Job Summary
The Home-Based Primary Care Nurse Practitioner delivers comprehensive, evidence-based geriatrics and extended care services within the HBPC program. Responsibilities include developing and implementing individualized care plans to optimize population health and disease management; leadership and collaboration with the interdisciplinary team; ordering and interpreting labs and diagnostic studies; prescribing pharmacologic and non-pharmacologic interventions; initiating referrals to specialty services; educating patients and families; adjusting the care plan to achieve optimal outcomes; delivering care in the home of Veterans with complex needs via home visits, Veteran Video Connection (VVC) visits, and telephone visits; and upholding ICARE values (Integrity, Commitment, Advocacy, Respect, Excellence). Preferred experience includes working with diverse Veterans, Geriatric and high-risk populations, with 2-3 years NP experience.
Required Qualifications
- Nurse Practitioner license
- National certification
- Licensed independent practitioner
